This privacy notice (“Privacy Notice”) outlines how DocForUs Private Limited ("DocForUs," "we," "us," or “our”) collects, uses, discloses, transfers, and otherwise processes personal data collected from users (“you” or “your”) through the DocForUs application/website.

Personal Data Processed

We may process the following categories of personal information for the corresponding purposes of providing you with our services, including:

Name, contact details such as phone number and email address, passwords, date of birth, and demographic information (e.g., age and gender)

Purpose:
To: (a) register users on the platform, (b) create and administer user accounts, and (c) authenticate logins.

User’s name, demographic information, medical information, including symptoms, information and documents related to past medical records such as diagnostic and lab reports, X-rays, prescriptions, discharge summaries, and any other medical records

Purpose:
To: (a) upload medical information (including through guided questions) to streamline information gathering before consultation with the doctor and (b) use AI tools to prompt guided questions, help users articulate concerns, and convert unstructured user inputs and uploaded records into structured outputs; (c) compile user concerns, symptoms, medical history, structured inputs, and uploaded documents and make them available to the treating doctor/hospital ahead of the consultation, (d) maintain and present prior consultation-related inputs and uploads so that the doctor has continuity across visits, and (e) retain your medical information and share it with your new doctor / hospital.

Contact information and information collected through device permissions

Purpose:
To: (a) send appointment-related alerts including follow-ups and health milestone nudges and (b) access files and images to facilitate upload of documents or images for storing the data and sharing with hospitals / doctors.

Name and registered contact details

Purpose:
To: (a) respond to queries, troubleshoot issues, manage user requests/complaints, (b) send service/administrative communications, and information that may be relevant or of interest to you, and (c) send marketing communications.

Data
Information from connected apps such as Apple Health or Strava (if collected, terms and conditions of such apps are also applicable)

Purpose:
To consolidate health information.

We may disclose a memory feature, which stores user information based on past conversations, connected apps or services to tailor responses.

Sharing of Personal Data with Third Parties

We may share your personal data with:

(i) Cloud service providers for the purpose of hosting and secure storage of personal data and Platform operations.

(ii) Government or law enforcement officials if we believe doing so is required to comply with law enforcement and legal process, to prevent or stop any illegal, unethical, or legally prohibited activity, and to protect your, our or others’ rights and safety.

(iii) A limited number of authorized personnel for providing customer support, troubleshooting issues, managing user requests or complaints, and ensuring the proper functioning and security of the platform.


Your Rights

Right to withdraw consent: You have the right to withdraw your consent at any time by contacting us through the Contact Us option. Please note that the withdrawal of consent may impact our ability to provide certain services, and we will inform you of any consequences upon withdrawal.

Right to access: Subject to certain conditions, and where we have relied on consent to process your personal data, or where you have voluntarily provided your personal data for a specified purpose, you have the right to obtain information from us as below, as well as such information as may be further prescribed under applicable law:

(i) Summary of personal data and processing activities: You have the right to receive a summary of the personal data that is being processed by us along with details regarding the processing activities undertaken by us with respect to such personal data.

(ii) Identities of other data fiduciaries and data processors: You have the right to know the identities of all other entities with whom your personal data has been shared by us. This includes a description of the personal data shared with these entities.

Right to correction, completion, and updation: Subject to certain conditions, you have the right to correction, completion, and updating of your personal data for which you have previously given consent or voluntarily provided your personal data for a specified purpose, in accordance with any requirement or procedure under any law for the time being in force. This includes the right to:

(i) Correct any inaccurate or misleading personal data.

(ii) Complete the incomplete personal data.

(iii) Update your personal data.

Right to erasure: Subject to certain conditions, you have the right to request the deletion of your personal data under certain circumstances, such as when the data is no longer necessary for the purposes for which it was collected or when you withdraw your consent.

Right to nomination: You have the right to nominate any other person to exercise the rights mentioned above in case of your death or incapacity.

Your Responsibilities You represent that all personal data provided during the registration process and while using our services is true, accurate, current, and complete. You are required to periodically review and update your registration data to ensure that it is always accurate and correct. We will rely on the last updated version of the personal data as updated by you. While providing your personal data, you agree to ensure not to impersonate another person, and to furnish only such information as is authentic.

The accuracy, integrity and completeness of the information you enter or allow to enter on your behalf is your responsibility, and we do not assume any liability for it.

User Control and Choices

The Privacy Notice clarifies that users have full control over their experience in Health: they can choose which third-party apps to connect or remove at any time through settings options, and they can manage how their memories are stored and used based on their personalization preferences.
